AUTHORIZATION TO USE VETERINARY CARE *

By checking this box, I agree that if my animal(s) experience a life-threatening emergency or other condition that in ABDB’s reasonable discretion requires immediate veterinarian care, ABDB may seek veterinarian care on your behalf, using a veterinarian of its choice. ABDB may authorize all services recommended by the veterinarian to save the life of your animal or otherwise treat the condition. You shall be responsible for all veterinarian costs incurred, and agree to indemnify and hold ABDB harmless for all of the costs.

BILLING & PAYMENTS *

By checking this box, I agree to pay all of the unpaid expenses my animal(s) have accrued while at ABDB as set forth in the “Rates and Policies” and in this “Boarding Agreement “. The full amount due at the time of pick-up or delivery must be paid at this time. No animal(s) will be allowed to leave ABDB’s care, custody, and control until the amount owed on the account is paid in full. Should the account go unpaid, ABDB reserves the right to enforce its lien rights against your animals under ORS 87.152 et seq.

DROP-OFF, PICK-UP, & VISITATION *

By checking this box, I agree that a scheduled appointment shall be necessary for drop-off, pick-up, or visitation.

GOOD HEALTH *

By checking this box, I agree that I represent that my animal(s) and any other animals surrendered to the care, custody, and control of ABDB are in good health, free of parasites, any other communicable disease diseases, and to have all current required inoculations.

LOSS & INJURY *

By checking this box, I agree that I hereby waive all rights and claims against ABDB and it’s agents and representatives for the loss, death, or injury of any animal(s) under its care, custody, and control, including but not limited to those resulting from accidents, disappearance, disease, fire, runaway, sickness, theft, whether during transportation or otherwise, whether on or off the premises of a BDB. This waiver includes any claims of negligence.

STATEMENT *

1) I have read and understand the ABDB “Boarding Agreement” and “Rates and Policies.”
2) I have the authority to enter into this Agreement as the legal owner, representative, or agent of the animal(s) being surrendered to ABDB.
3) I accept all terms and conditions of the Agreement and the “Rates and Policies.”
4) I agree to be bound by the terms and conditions outlined in both the Agreement and the “Rates and Policies.”
